Profile of Alcohol-Related Liver Disease Among Patients Attending Tertiary Care Hospital in India — Insights from a Study in North Western India

Round pneumonia in adults masquerading as bronchogenic carcinoma

Lemierre's Syndrome — a rare fusobacterial complication

A gastroenterologist specializes in disorders of the digestive system, including the esophagus, stomach, small intestine, large intestine (colon), liver, pancreas, gallbladder, and bile ducts. Common conditions include GERD, peptic ulcers, inflammatory bowel disease (Crohn's, UC), irritable bowel syndrome, liver diseases (hepatitis, cirrhosis, fatty liver), colorectal polyps and cancer, celiac disease, pancreatitis, and GI bleeding.
You should consult a gastroenterologist if you experience persistent symptoms such as: chronic abdominal pain or bloating, frequent heartburn not responding to over-the-counter medication, difficulty swallowing, unexplained weight loss, blood in stool or black/tarry stools, persistent changes in bowel habits, recurrent nausea or vomiting, or abnormal liver function test results. Individuals above 45 should also consider a colonoscopy for colorectal cancer screening.
An upper GI endoscopy (EGD) is a procedure where a thin, flexible camera is passed through the mouth to examine the esophagus, stomach, and duodenum. It is performed under sedation or analgesia, so patients experience minimal discomfort. The procedure typically takes 15–20 minutes. It is used to diagnose ulcers, GERD complications, Barrett's esophagus, celiac disease, and to take biopsies when needed.
PPI-resistant dyspepsia refers to symptoms of indigestion, upper abdominal discomfort, or bloating that persist despite a standard course of Proton Pump Inhibitors (PPIs like omeprazole or pantoprazole). Dr. Arjun Mehta's DM research specifically focused on this condition. Treatment involves thorough endoscopic evaluation to identify underlying causes (H. pylori, erosive gastritis, motility disorders), followed by targeted therapy which may include H. pylori eradication, prokinetics, antidepressants for functional dyspepsia, or dietary modifications.
Colonoscopy preparation involves bowel cleansing to ensure a clear view of the colon. Typically: 2 days before — adopt a low-fibre diet and avoid seeds, nuts, or raw vegetables. The day before — drink only clear liquids (water, clear juices, broth) and consume the prescribed bowel prep solution in split doses. Avoid solid foods. On the day — continue clear liquids until 3–4 hours before the procedure. Bring a companion as sedation is given and you will not be able to drive after the procedure.
Non-Alcoholic Fatty Liver Disease (NAFLD) is the accumulation of excess fat in liver cells in people who drink little or no alcohol. It ranges from simple steatosis (reversible) to NASH (non-alcoholic steatohepatitis with inflammation) and cirrhosis. Early stages can be reversed through: 5–10% weight reduction, Mediterranean diet, regular aerobic exercise, controlling diabetes, blood pressure, and cholesterol. Advanced stages require specialist monitoring and treatment to prevent progression to liver failure.
IBS (Irritable Bowel Syndrome) is a functional disorder — the bowel appears normal structurally but functions abnormally, causing abdominal pain, bloating, and alternating constipation/diarrhea. It does not cause inflammation or structural damage. IBD (Inflammatory Bowel Disease) — comprising Crohn's disease and Ulcerative Colitis — is an organic condition involving actual inflammation and ulceration of the intestinal wall, which can be seen on endoscopy and biopsy. IBD can lead to serious complications and typically requires long-term medical or surgical management.
For average-risk individuals, a colonoscopy is recommended starting at age 45–50, then every 10 years if normal. If polyps are found, follow-up may be needed in 3–5 years depending on the type and number. For high-risk individuals (family history of colorectal cancer, personal history of polyps, IBD, or hereditary syndromes like Lynch syndrome), screening should begin earlier and occur more frequently.
